Social Services

    Each facility has a Social Services Director to assist residents and their families with admission and adjustment to the care center and with on-going needs or concerns related to the care and services provided at the centers as well as discharge planning when a resident is ready to return home.

    The Social Services Director assesses each resident’s psychosocial needs and develops a plan to see that these needs are met.  The Social Services Director also provides education materials, special programs and support groups.  Special family events provide socialization, education and entertainment.  Each care center has an active Resident Council which provides residents the opportunity to express their interests and concerns.
