Location

    Crossroads Care Center and South Shore Care Center are conveniently located in Worthington, Minnesota.  Major referral sources are the Avera Worthington Specialty Clinics, Sanford Clinic Worthington, Sanford Regional Hospital Worthington, McKennan and Sanford Hospitals in Sioux Falls, South Dakota and Mayo Clinic and its affiliated hospitals in Rochester, Minnesota.  Interstate 90 and Highways 59 and 60 provide easy access to Worthington.

    Both facilities are located in quiet, residential areas close to downtown Worthington.  Two van services provide non-emergency transportation to local or out of town medical appointments.  A taxi service is also available to provide transportation throughout the city.  The facilities own a 20 passenger bus which is used to transport residents on social excursions.

    Worthington is a regional center for medical services.  The local hospital provides a number of specialized services including coronary care, orthopedic surgery, dialysis and radiation services.  The proximity of Crossroads and South Shore Care Centers to these services makes our facilities ideal locations for patients who need skilled, convalescent or rehabilitative care following a major illness, surgery, dialysis and other treatments.
